Brisbane has booked their spot in the Grand Final, coming from 25 points down to overrun Geelong.
The Cats looked to have things in control before the Lions, for the second straight week, pulled a rabbit out of the hat.
The injury to Oscar McInerney now looms over the Grand Final against Sydney, with the ruckman dislocating his shoulder.
See our grades for every player from the match below:
Gryan Miers - A+
What an unreal Preliminary Final performance. Miers not only turned low percentage moments into goal assists, he finished a few himself too.
Oliver Henry - A
Kicked four in a Preliminary Final, including what could have been the match-winner late in the fourth term.
Tom Stewart - A
Had Geelong won, his chase-down tackle from behind late would have been the moment remembered forever. Still had an enormous impact across half-back and through the midfield.
Lawson Humphries - A-
Trusted with kick outs and used the ball well off half back. Turned what could've been a Brisbane goal into a 100m rebound in the opposite direction in the first half. The Cats have found a serious player here.
Jeremy Cameron - B+
Kicked 2.3... will probably wonder what could've been with a few of those shots for goal, but couldn't have done much more.
Zach Guthrie - B+
Another strong finals performance from Guthrie. 18 disposals and 7 marks playing across half-back.
Rhys Stanley - B
Hit the post from the top of the goal square in the final minute. Probably wouldn't have mattered, but it's all people will remember despite what was a solid performance.
Mark Blicavs - B
Had the job on Joe Daniher, while also spending some time in the ruck. Played more midfield in the second half.
Jed Bews - B
Had the unenviable task of minding Charlie Cameron, who will always get you at some stage, regardless of how well you defend. Did the job for the most part.
Patrick Dangerfield - B
Won a few big clearances when the Cats needed them, but looked restricted with back soreness at times.
Shaun Mannagh - B-
Once again did not look out of place on an enormous stage. Not quite the performance of weeks ago, though.
Max Holmes - B-
Missed the 2022 Grand Final with a soft tissue injury and suffered another one in a Prelim. You've got to feel for him, especially since he was absolutely on fire in the first half of this one.
Brad Close - C+
Jack Henry - C+
Spent the majority of the evening on Logan Morris, who kicked two - one he could do nothing about.
Jake Kolodjashnij - C+
Jack Bowes - C
Tyson Stengle - C
Six score involvements, but didn't kick a goal himself. Well held in the end.
Oisin Mullin - C-
Tanner Bruhn - C-
Pretty quiet. A lot asked of a young midfielder who wasn't quite ready for the moment.
Oliver Dempsey - C-
Tom Atkins - D+
Attempted to lock down Neale, but didn't have much of a say himself - and Neale got off the chain in the second half.
Shannon Neale - D
Largely invisible outside of bringing a few contests to ground. Had a shot for goal from 50 in the fourth term that ultimately didn't make the distance.
Mitch Duncan - N/A
Subbed on in the fourth quarter for Max Holmes.
Callum Ah Chee - A+
Struggled to limit Tom Stewart's impact in the first half in a defensive forward role, but wowee did he have an impact in the 2nd half: kicked a big goal early in the third to keep the Lions in it and then another in the fourth to give them the lead and then a third with two minutes left to put them in front again.
Hugh McCluggage - A+
Made two big mistakes in the first quarter, but was unreal from that point onwards. Helped bring the Lions back into the contest with his work at stoppage.
Lachie Neale - A
An enormous third quarter from Neale helped lift the Lions back into the contest. He is clearly fighting through pain, but lifted as all good champions do.
Joe Daniher - A
You might be surprised by the A given Daniher went goalless, but Daniher's ability to go full-time into the ruck when McInerney went down and to remain competitive kept the Lions in it when the game was starting to look out of reach. Was also seen deep in defence. Deserves credit.
Darcy Wilmot - A
Provided a ton of dash off half-back. Did not look out of place on the big stage once again. A great contribution on the outside and also around the contest.
Dayne Zorko - A
Made a number of mistakes by foot trying to take the game on and make things happen. Far from his cleanest performance, but certainly helped spark the comeback in the third term.
Brandon Starcevich - A
Kept Stengle goalless ultimately, on top of his toughness in multiple contests.
Cam Rayner - B+
Two goals from outside 50... one in the final 100 seconds to put them two goals up. A massive mark in the final term too. A man for the big moments.
Jack Payne - B+
Spent most of the evening on Shannon Neale, allowing Andrews to play his usual intercepting role. Laid the big tackle on Mitch Duncan to save a goal in the final minute. What an effort playing through the knee injury.
Noah Answerth - B
Crucial clearance which led to a late Lions goal. One of his better performances for the club.
Kai Lohmann - B
When Brisbane looked down and out, it was Lohmann who sparked them in the third quarter as much as anybody else.
Ryan Lester - B
Had the enormous task of playing on Jeremy Cameron and was beaten soundly in the first half. Was a part of multiple goal-saving contests in the second half however.
Charlie Cameron - B
Kicked two big goals and set up Ah Chee in the fourth term. Took his moments when they were there.
Harris Andrews - B
Mostly blanketed Shannon Neale and Ollie Henry in the air, but the speed at which the Cats attacked made it nearly impossible for him to intercept at times.
Will Ashcroft - B
The second-year midfielder did not look out of place, winning 22 disposals. Wasn't quite as clean by foot as the previous two weeks, but such was the pressure of a prelim.
Zac Bailey - B
Kicked two goals that ultimately helped decide the result, while also spraying two that would have changed the momentum of the game early.
Josh Dunkley - B
Found 28 disposals, 10 contested, but only won the one clearance and laid the four tackles. Not his biggest night.
Jarrod Berry - B-
Was BOG one week ago. Not as influential in the prelim. Still found 20 disposals, spending more time on the wing.
Logan Morris - B-
Not a high possession player, but impacted in moments, kicking two goals and taking five marks.
Eric Hipwood - C+
Jaspa Fletcher - C
Conor McKenna - N/A
Subbed on for McInerney and invented a goal from nothing in the fourth term, setting up Morris in the goal square.
Oscar McInerney - N/A
Had control early before dislocating his shoulder late in the first quarter. Courageously battled on with one good arm to even up the ruck battle, but it popped out once again in the third term. Deserves enormous kudos for trying to play on.
