The minor premiers are through to the Grand Final, putting away the Port Adelaide by 36 points at the SCG in a dominant Preliminary Final.
The early affairs went the way of the home side as the typically slow-starting Swans slammed four first quarter goals despite a Power surge at the centre of the SCG.
Sydney's stand-in captain Dane Rampe may have put his September in jeopardy with his late hit on Zak Butters midway through the first term.
Sydney gained complete control as the contest played out, owning the uncontested footy and progressing down the ground where the desperately demanded key forward stocks of Logan McDonald and Joel Amartey began to deliver.
Port Adelaide’s attempts to wrestle back any momentum were stunted by a Swans outfit clearly up for the fight, as Sydney entered the main break with a healthy 25-point lead despite the Power recording +5 inside fifties.
It was all Bloods as the second half commenced, blowing out the margin to 40 points with 12 of their first 13 goals spawning directly from turnovers, including this charging effort by Chad Warner.
A professional final term from John Longmire's men saw the Swans hold on to a phenomenal victory as they book their place in the big dance, claiming a dominant 14.11 (95) to 8.11 (59) win.

Isaac Heeney: A+
Not entirely sure that there's a way to stop this guy, particularly within this run of form. Had Willem Drew for company all night and yet wasn't a step off the pace. 200 games in and playing his best career footy. 24 disposals, six tackles, eight score involvements and two goals.
Chad Warner: A
As lively as ever. Two brilliant goals to put away the Power, 21 touches with great use to go with five clearances. Carried last week's final term form into this one with ease.
Errol Gulden: A
Labelled mid-week by Champion Data as a ‘must tag’, ran loose and made Port pay. The best user in the game, and didn't he show it. 27 disposals, six involvements.
Oliver Florent: A
Superb. Plenty of touch on the outer side, very rarely misused it.
Joel Amartey: A-
Just as Sydney was screaming for key stocks. Showed plenty of signs throughout the year, put it all together tonight. If he can carry that level of contest into next week, finding the sticks and pressuring in the front half, it's hard to count the Swans out.
Logan McDonald: B+
Wasn’t deployed forward until the second half in the Qualifying Final in a contest that screamed for a key target. In the front 50 from the jump and seemed the answer to all the Swans’ questions. Fingers crossed the late ankle complaint is not too much of a major concern.
Dane Rampe: B+
The Swans will be praying that fortunes fall in Rampe's favour following his late hit on Butters. Played a fantastic game in his 250th appearance, managed a team-high seven intercepts, was composed all night.
Nick Blakey: B+
Pounced on the footy all night, will never miss the chance to run.
Tom Papley: B+
Eight disposals but three goals. Made the most of every opportunity despite seemingly battling a persisting injury.
Tom McCartin: B
Justin McInerney: B
Elite run proved so vital on numerous occasions. Building in confidence following time out, will definitely hope to put it all together in the big dance.
James Jordan: B
Was given the task on Butters, held him well. Will certainly have a task next week, expect Tom Stewart or Dayne Zorko. A fantastic tool at Longmire's disposal.
James Rowbottom: B
A typical James Rowbottom game. Double digit tackles.
Jake Lloyd: B
Luke Parker: B-
Shifted forward to take a role on Aliir Aliir and did well. Slowed the defender's ability to intercept, as he was so freely in the first term.
Will Hayward: B-
Took a few massive marks to relieve the back half. A great role.
Hayden McLean: B-

Brodie Grundy: B-
Slightly edged out by Sweet. Not a bad game by any means.

Started as well as anyone, intercepting with ease. Was targeted heavily as the game progressed, still amounted a whopping 14 intercepts.

A totally dominant start against very talented opposition in Grundy. Slowed late, credited as the stronger of the ruckman. Six clearances.
Jason Horne-Francis: B+
A charging first term but was well held late. 23 touches and seven clearances.
Jase Burgoyne: B
Connor Rozee: B
Lots of uncontested ball, great run and hit the scoreboard. Led the game in inside 50's.
Ryan Burton: B
A welcomed inclusion, plenty of forward ball.

Handed an impossible job on Isaac Heeney, was not able to do much. Still, had a decent game in his own right.
Zak Butters: B-
Held really well by James Jordan.

Certainly enhanced his involvement. 12 and a goal.
Travis Boak: C+
Feel for Boak as he did so much right, got himself into great positions and claimed the ball well. Unfortunately, his disposal let him down to astronomical effect. Seven turnovers, some of which were entirely costly when the game was still somewhat in the balance.
Willie Rioli: C+
Only needs half a chance to nail a major. Unfortunately, Rioli was only afforded half-chances all night.
